2017-10-17 How the TSB investigates for Human and Organizational Factors

After a great kick-off to CapCHI’s 2017-18 program in September, our October will take a bit of a shift. This month, we’re presenting Michelle Gauthier, Senior Human Factors Investigator at the Transportation Safety board of Canada (TSB). Michelle will be discussing techniques used as well as some case studies in her discussion titled, “How the TSB investigates for Human and Organizational Factors”.

We’re back to our regular spot on the third floor at The Fox & Feather Pub & Grill on October 17. Doors open at 6:00pm for networking and the event starts at 6:30pm. Presentation Synopsis:

How the TSB investigates for Human and Organizational Factors

Human factors are involved in an estimated 60 to 90 percent of all transportation accidents and incidents. The importance of human factors as an element in transportation occurrences has been recognized throughout the world by governments and industry. In order to understand “why” an accident or incident occurred, the human factors of the occurrence must be investigated fully.

Speaker bio:

Michelle Gauthier, a Senior Human Factors Investigator with the Transportation Safety Board (TSB), will present how the TSB investigates for human and organizational factors. She will explain how a systematic approach to understanding the role human factors play in occurrences. Several recent incidents will be presented including two of Canada’s largest rail accidents that occurred in 2013; the crash between an OC Transpo double-decker bus and a VIA passenger train and the Lac Megantic rail disaster.
